About Us
Primary Care Networks (PCN) are a new initiative in the NHS designed to improve the quality of care outside of hospital.
A group of GP practices working with community providers develop services at scale for the residents of a defined area. Funding is available to PCNs to develop new services and employ new members of staff such as Clinical Pharmacists, First Contact Physiotherapists, Paramedics, Dieticians, Social Prescribers and more.
Working together, our 6 Purbeck PCN practices, (Bere Regis, Corfe Castle, Sandford, Swanage, Wareham and Wellbridge) along with Dorset Healthcare University NHS Foundation Trust, Mental Health Services and Community Voluntary services are looking at new ways to support Purbeck residents with a range of physical & mental health issues, as well as promoting health activities & wellbeing.
Purbeck PCN aims to build on some of the success achieved since it’s inception in 2019 and will continue to explore new opportunities, developing new & innovative services for the residents of Purbeck.
As Purbeck residents are at the heart of any new services the Network delivers, we have been and will continue to work closely with the Practice Patient Participation Groups (PPG’s) and Youth Patient Participation Groups (YPPG’s). These groups provide feedback from patient representatives to help shape our projects, ensuring they deliver what Purbeck Residents both want & need.
You may start to see changes in the way that existing services are delivered or be offered new services or new ways of interacting with health professionals. As these services develop, we will continue to involve Purbeck residents in evaluating their impact. We look forward to your continuing support as we move forward together.

What is a Primary Care Network?
A Primary Care Network is a group of GP practices who work together with community, mental health, social care, and voluntary services in the local area, to provide comprehensive primary care services to a defined population.
PCNs build on existing primary care services and enable greater provision of proactive, personalised, and coordinated health and social care for people close to home.
Over 99% of general practices are part of a PCN who sign up to the Network Contract DES which details our core requirements and entitlements.
What benefits do Primary Care Networks have for patients?
- They offer a larger range of care services that well be close to patients' homes, as well as improved access
- PCNs work with a wider range of health and community services, for example Bromley GP Alliance and Bromley Healthcare
- Patients will be able to receive support for a more complicated conditions, and will have access to the health and care services that can support them
- Patients will be able to have more of a role in making decisions around their own health and the care they receive What can you expect from your PCN? Additional healthcare professionals are being recruited to work on behalf of our GP practices within the Primary Care Network. These roles include Clinical Pharmacists, Mental Health Practitioners, Physiotherapists, Dietitians, Podiatrists, Social Prescribing Link Workers and Care Coordinators. By introducing a greater skill mix, our patients will have greater access to the support they need to keep people healthy and independent.
